Acknowledgment of Risk, Waiver, and Release of Liability Agreement
This waiver must be completed and signed for each individual who wishes to participate in the following activities at Sandy Cove: Rock Climbing Wall, High Ropes Course Elements, Zip Line and Giant Swing. Please bring the completed form with you.
You must be at least 10 years of age to climb the wall, and at least 12 years old to participate in the Zip Line and High Ropes Course. A parent or legal guardian must sign the waiver for those under 18. More than one child? You may list all your children's names on one form.
